Who operates this website

AQIMERYL is operated by Nuvanitic IntelliHealth Inc. (referred to as “AQIMERYL,” “we,” or “us”). This policy describes personal information collected through the AQIMERYL website, contact forms and Neighbourhood Evidence Brief request process.

Website: https://aqimeryl.biosyntel.com/ (and any AQIMERYL domain that expressly links to this policy).

Information we collect

When you use our contact form, we may collect your name, email address, organization, area of interest, referral source and the message you choose to send.

When you use our Tally-hosted Evidence Brief request form, we may collect your name, role, work contact details, organization, geographic area, planning question, project context and optional files you choose to upload.

Our hosting and form providers may process technical information, such as IP address, browser details, device information, timestamps and pages requested, to provide services, maintain security and measure site performance.

Please do not submit patient records, identifiable personal health information or sensitive details about individual service users.

How information is used and the basis for processing

We use submitted information to respond to inquiries; assess feasibility; clarify a requested Evidence Brief; provide a scope or quotation; prepare agreed work; keep necessary business records; protect the website; and meet applicable legal obligations.

Submitting a form is voluntary. We ask for information relevant to the stated purpose and, where required, rely on appropriate consent or another legally permitted basis for its collection, use and disclosure. Using the website alone is not treated as consent to unrelated purposes.

Service providers, access and disclosure

Our website uses Netlify hosting and Netlify Forms for general inquiries. The Evidence Brief request form is provided by Tally. Email providers may process correspondence. These providers may process information, including outside Canada, according to their applicable services and privacy terms.

Access to inquiries and uploaded materials should be restricted to authorized AQIMERYL personnel or service providers who need them for intake, scoping, reporting or support. We do not sell personal information. We may disclose information if legally required or where reasonably necessary to protect legal rights.

Provider information: Netlify privacy policy and Tally privacy policy.

Files uploaded through Tally

Optional attachments are intended for authorized organizational reference materials that help explain a planning question. Do not upload identifiable patient information or confidential records without a separately agreed and suitable process.

Uploaded files can be viewed or downloaded from the Tally form account by authorized account users. If downloaded for a project, copies may be kept in AQIMERYL's restricted working records and should be covered by the same retention and deletion rules.

Attachments are to be deleted when no longer needed for the relevant inquiry or engagement, subject to legitimate recordkeeping and legal obligations. Tally may retain deleted submissions in its Trash for up to 90 days unless Trash is cleared earlier. Provider backups and technical logs are governed by the relevant provider's practices.

Retention and deletion

Our stated general retention limit is five years from the last substantive interaction or completion of the relevant engagement, unless a shorter period is appropriate or longer retention is required by law, a documented legal hold or an applicable agreement. Unnecessary files should be removed sooner.

This is an organizational retention commitment, not a claim that the website or Tally automatically deletes records after five years. AQIMERYL must implement manual deletion procedures or configure supported service retention settings to meet the commitment.

On a valid deletion request, we will assess which information can be removed and what must be retained for legitimate legal or recordkeeping reasons. Deletion from third-party systems, their Trash and backups may occur on different schedules.

Cookies, analytics and technical records

The current static website package does not include a Google Analytics tag, advertising pixel or other optional marketing-tracking script identified in its HTML. We have not verified account-level Netlify analytics settings or every third-party service configuration.

Website hosting may produce routine technical access and security logs. Embedded Tally forms and linked third-party services may use cookies or comparable browser storage under their own policies.

If AQIMERYL enables optional analytics or advertising technologies, this policy and any required consent controls should be updated before that use begins. Users can manage cookies through their browser settings.

Canadian privacy law and health information

For commercial collection, use and disclosure of personal information, Canada's Personal Information Protection and Electronic Documents Act (PIPEDA) generally applies to private-sector organizations operating in Ontario, subject to the particular circumstances and other applicable laws.

Ontario's Personal Health Information Protection Act (PHIPA) has distinct requirements for health information custodians and certain agents or service providers. AQIMERYL's ordinary website forms are not intended to collect personal health information. Any project involving such information would require a separate assessment, documented safeguards and appropriate agreements before data are received.

Other privacy laws may apply depending on an individual's location and the nature of the activity; this notice does not assert that GDPR or UK GDPR automatically applies to every visitor.

Security

We use third-party service infrastructure and aim to restrict access to information according to operational need. We cannot guarantee absolute security of information transmitted over the internet.

Please contact the privacy email below before sending information you believe may require special handling.

Your privacy choices

You may contact us to request access to personal information about you, correction of inaccurate information or deletion where applicable, or to raise concerns about our handling of your information. We may need to verify your identity before processing a request.

Children

The website and Evidence Brief intake are designed for organizational and professional use. We do not knowingly solicit information from children under 13; a parent or guardian can contact our privacy office if they believe a child has submitted information.

Changes to this policy

We may revise this policy as practices, services or legal requirements change. The effective date above will be updated when a revision takes effect. Material changes will be communicated through the website or another appropriate channel when required.
